Forms and Guidelines for Nonprofits 

Are you a CFBC grantee?

Congratulations on receiving a grant from the Community Foundation of Boone County! Please read the following information in detail. If you have any questions, please reach out.

Required Reporting

Organizations that receive grants and wish to apply again must first submit a final report for any previously funded projects or programs in order to be eligible for funds again. The final report deadline will be stated in the Grant Agreement; most CFBC grants require the report to be submitted within 30 days after the end of the grant period. Please complete the final report through our online portal.

Requesting a Grant Extension

Grants that are unable to spend the entire amount awarded or complete a majority of the funded activity within the funded grant period are required to either refund the remaining balance with an explanation or submit a grant period extension request. Email our President & CEO, Julie Reagan, explaining the reason for requesting the grant extension, why you were unable to use the funds within the original timeframe, and your plan to use all grant dollars by the extended deadline if your extension request is approved.
